Original seems fine. Change implies it occurs at other time frames following surgery. Can it occur outside of hours after surgery?

Immediate postsurgical fever which occurs within hours of the operation can be due to what causes?
Might be having a brain fart, but I feel both work grammatically. Fine with either
I think it’s just that the original, though gramatically correct, sounds awkward + is a mouthful? idk
Might actually be the end of sentence tripping us up? Formatting as the following sounds a lot better to me:
