This card always bugged me
even the new changes wont help you answer UW questions about RCRI
Yeah I missed a question yesterday where you had to actually calculate their RCRI to know what needed to be done next or something. What changes would you make?
A good change in my opinion would be to add ECG to the question “beside ECG…” and keep stress test and echo as two answer
in the extra section
- If the Revised Cardiac Risk Index (RCRI) is ≤ 1 proceed with surgery without further testing
- if RCRI ≥ 2 further testing is warranted cardiac stress testing (e.g., exercise or pharmacologic)
- echo is indicated if the patient has a history of uncompensated HF or valvular disease"

and btw, i believe this is a very HY topic, in my last shelf exam i felt like all the questions were pre and post op qs lol
I agree that this card still needs some more work, but in the mean time we should remove “Cath” from text field.
I think it’s worth discussing making a one-by-one card for the RCRI criteria because I have seen at least 1 question where you needed to know them, probably more questions than that too.

I still believe the first two extra notes need some revision.
i am thinking sth like this in the extra section
- No history of cardiac disease or risk factors → No routine preoperative cardiac testing needed
- ECG is indicated for arrhythmia, ASCVD, structural heart disease, or high-risk surgery
- Echocardiogram is indicated for heart failure, unexplained dyspnea, valvular disease, or pulmonary hypertension
- Stress Test is considerd if RCRI ≥ 2 and poor functional capacity (<4 METs)

Cath is very, very wrong and not recommended for pre-op eval. really only done if patient is having an acute MI
most questions the answer is “no further testing indicated” anyways
