In my AMBOSS Renal block I’m doing now, PSGN (nephritic) and MCD (nephrotic) have both been correct answers to clinical presentations involving periorbital edema as the major physical exam finding. In both question stems, the HPI made it obvious which renal syndrome was being asked about (hx skin infection for PSGN, child with severe proteinuria for MCD). I stand by my statement that PSGN seems to be the only nephritic syndrome associated with periorbital edema that I’ve come across. Some primary sources just vaguely say “edema” in the clinical features of PSGN and nephritic syndrome. I’d also be ok with a note in the Extra saying “nephrotic syndrome is also associated with periorbital edema” or “PSGN specifically is associated with periorbital edema” or just remove the specifier and ask about edema. Or whatever y’all think is best.
Think card is straight from Pathoma and current Pathoma text still includes this. I think if we do decide to change this it’s worth an explanation as to why in Extra.
That said, if we are still seeing it described with some cases of nephritic on question stems it’s worth keeping here. Thinking a note in Extra saying it is also seen w/ nephrotic like suggested above may be a better move as typically there will be a ton of other details in those questions pointing you either way.
