I guess my point is that this card is unclear as is. A handlebar injury can cause injury to the liver, spleen, or pancreas commonly, but delayed onset of symptoms following initial negative workup is the tip off for a pancreatic injury over a liver or spleen injury, which would generally be more common

The liver and spleen are the most commonly injured solid organs in blunt abdominal trauma. Although both hepatic laceration and splenic rupture can generate intraabdominal free fluid (ie, blood), injuries severe enough to cause a large amount of intraabdominal bleeding (eg, large amount of free fluid) are nearly always detected on initial abdominal CT scan

Another option could be:

Persistent abdominal pain, nausea, fever, and free fluid in the upper abdomen days after blunt abdominal trauma with an initially negative CT suggests {{c1::pancreatic rupture / laceration}}
