I believe both are acceptable locations depending on the resource you are using. Not sure about the discrepancy between the image and card.

@hisham242 is correct, I believe there is a discrepancy between major resources and that’s how this issue was introduced

for the cloze/exams left lower sternal border (LLSB) should be enough, 4th or 5th can be mentioned in the extra section
