Will let another maintainer weigh in, but I think the extra of the card already explains what you think is missing on this card. The card is meant to test physical exam findings seen in SVC syndrome. JVD tells you there’s impaired drainage and facial plethora tells you it’s resulting in edema of the head/neck. Don’t think anything needs to change here.
Either seems fine. Extra mentions those other points like @Cameron said.
The point isn’t what you see on extra, it’s what you see before you flip the card over. If each card is supposed to be a tes of your recall, then it should be prompting you for physical symptoms and be precise about the way the question is phrased.
