I can also confirm that the Amboss question is about a patient with low grade dysplasia and the correct answer is ablation (no option that says surveillance)

But from my quick reading, I think it should be “endoscopic radiofrequency ablation OR surveillance endoscopy q6-12 months” as they are alternative options to each other

Another important thing is that we should “ideally” add “PPIs” to the question “Beside PPIs, what is the next step”

Content error with 2 suggestions open to fix this.
I edited this suggestion because changing both low-grade and high-grade to the same thing would make this kinda redundant.

I got it right after going back and forth before deciding that waiting 2 years was too long. Plus question stem never said PPIs were started so no way you gonna wait 24 months without giving high dose protonix

UW Step 3 QID 5070: Patients with low-grade dysplasia require surveillance endoscopy every 6-12 months or endoscopic eradication therapy (eg, ablation, mucosal resection).

I think new phrasing is correct, and would add ablation as alternative in text field. Agree with addition of PPIs. ACG guidelines below

NBME Surgery Form 8 Q25 tests this as well, gives patient with Barret esophagus and mild dysplasia on biopsy with answer choices including both 6 month surveillance vs ablation and 6 month surveillance was the correct answer.

Their explanation for why ablation was wrong was that it may be necessary if repeat biopsy shows dysplasia, but that repeat biopsy was the initial requisite step :roll_eyes:

NBME forms are retired questions. Doing a bunch right now for Step 2 and have to remind myself that.

Constantly missing OA treatment questions because I pick NSAIDs, but Step 2 Forms all say acetaminophen is preferred. This is outdated guideline and recent guidelines have NSAIDs as first-line (which is what step 2 now tests on)
I just ignore those questions when I get them “wrong”
