These are called DOACs/NOACs and are now in firstline. However warfarin is still prefered in many other conditions like DVT due to cancer, or very high thrombosis load etc… can add that in extra maybe?

I think it is still debatable. For cancer it used to be LMWH. We can write that rn most of the time it is either DOACs or Heparins. Think of VKA if renal insufficiency
