While AMBOSS agrees in their notes section (“Contrary to what the term suggests, scleral icterus is caused by the accumulation of bilirubin in the conjunctiva and not in the sclera.”), they still call it scleral icterus in most articles and questions I’ve seen. I lean towards keeping as-is, but I could also see adding this distinction in the Extra.
