this UW QID is a young female with the sudden onset eye redness (consistent with subconjunctival hemorrhage) + typical metabolic abnormalities of bulimia nervosa. “This patient with a subconjunctival hemorrhage (SH) also has signs of gastrointestinal fluid loss (ie, muscle cramps possibly due to hypokalemia, alkalosis with chloride depletion), raising concern for repeated vomiting. SHs may be a result of direct trauma (eg, eye rubbing) but may also result from rapid increases in intraabdominal pressure (eg, vomiting). In an otherwise healthy, young patient, self-induced vomiting (ie, purging behavior) should be suspected.”
