@bginsy95 just to clarify, does the question require you to pick MRI/MRV or to know that that would be the next step? Primarily a Step 2 card so would be reticent to add a Step 1 tag unless it’s hyper specific to this UW question
19yo female presenting with obesity, comes in with bifrontal headache’s and early morning nausea and vomiting for 4 weeks. appears alert and cooperative. BP, HR are both in within normal range, bmi was obese. fundoscopy was 20/20 bilaterally, neuro exam had no abnormalities, Ct of head revealed nothing.
The answer to the question was a cerebral venous hypertension. It could probably do better for step 2 tagging but the question states that the CT of the head showed no abnormalities. In both idiopathic intracranial hypertension and cerebral venous hypertension, they would show a normal CT of the head. So i was under the assumption that it couldve helped with answering the question because of knowing that you would expect the CT to be normal while the MRI/MRV would be abnormal instead.
It could be better for step 2, but it might have helped with answering the question if a normal CT wouldve ruled in options while also ruling out other options. maybe even in the extra adding “CT would be normal.”

Ahh I see, all good. MRI is used more to verify that performing an LP is safe and not so much used to diagnose/find evidence of IIH (it can also be grossly normal on MRI!). Don’t think this card would be helpful for the question, but thanks for the suggestion ![]()
