This is restating the same thing; prefer the original personally but don’t think it would hurt to spell out the effect of low Ca/Vit D, and high PO4 by adding this to a second bullet

Low Ca/Vit D is what causes continuous stimulation of those receptors and consequent downregulation afaik

I just didn’t like the previous explanation because it makes it sound like tertiary hyperparathyroidism presents with low calcium, but it presents with high calcium
