This card to me is testing on concept of patient presents with most likely hemorrhoids what do you do? Both UWorld and AMBOSS say age over 50 y/o or red flag sx of CRC = colonoscopy.

This card originally said 50 years old but was changed to 45 years old based on CRC screening guidelines. Again I think the intent of this card was originally for hemorrhoids and not CRC. In patients over 50 even if you see a massive/feel hemorrhoid on DRE you do not stop there and you get a colonoscopy even though you have a diagnosis already. (due to age it’s better to be safe and check anyway) If it’s a younger patient you can stop and just treat hemorrhoid.

UWorld QID 3857 contradicts what AMBOSS states with age > 50 years old.

“Colonoscopy should also be considered for those age ≥45, for whom colorectal cancer screening via colonoscopy is appropriate regardless of symptoms.”

In this question the patient was 36 so colonoscopy was incorrect but above is explanation why it’s not indicated
