What was the answer to this question (Step 1 2102) ? Was it directly related to aerobic vs anaerobic?

Regardless, I think its time to change this

Support, you also dont need to exclude anaerobic bacteria completely

Just did Step 2 4118 a few minutes ago and the explanation includes the same info re: common causative organisms (question is about risk factors for aspiration tho)

Tried making the note a bit more concise (generally feel our content should speak for itself without necessarily having to reference to AMBOSS/UW):

This card used to say anaerobic; newer studies show that aerobic upper respiratory organisms are the most common cause. Anaerobes are more likely present in the setting of lung abscesses or empyema (Sep 2024)

vs

According to UWorld, previous studies indicated that anaerobic bacteria were responsible. However, newer studies show that aerobic upper respiratory organisms most likely cause aspiration pneumonia. This means anaerobic coverage is only needed in lung abscesses or empyema (August 2024)

I swear i’ve seen question explanations say aspiration pna is usually polymicrobial (aerobic and anaerobic)

Yeah I have a UW missed questions screenshot on this exact card from 2023

what I put was it can be from a “mixture of upper respiratory aerobes and oral cavity anaerobes”

AMBOSS says " Aspiration pneumonia a type of pneumonia that develops following aspiration of oropharyngeal secretions and/or gastric contents."

Updated suggestion to address @Cameron and @IantheBFG 's concerns. AMBOSS also says mixed aerobic and anaerobic can occur but that’s 3rd on their list - added that to the extra section

@Ahmed7 Uword 2102 does not directly test aerobic vs anaerobic but the explanation says that aerobic is the main cause unless there is a complication such as an abscess should anaerobic coverage be added

Thank you! changed my vote to :+1:

Looks great now, but I have a nitpicky suggestion lol

I like moving the second part of change explanation to it’s own line because it’s not related to why we changed the card. Also we usually have UW or AMBOSS specifically named in the reasons we change clozes so added that.

Infections may still be polymicrobial, containing both aerobic and anaerobic pathogens

Aspiration pneumonia complications, such as lung abscesses or empyema, are usually polymicrobial

This card used to say anaerobic; however, AMBOSS and recent studies show that aerobic upper respiratory organisms are the most common cause (Sep 2024)

@IantheBFG That sounds good to me, just updated the suggestion
