amboss does not mention shock in hypoplastic LH
Inadequate organ perfusion is the defining feature of shock. Amboss indicates “survival depends on presence of PDA” as an “open PDA provides adequate systemic perfusion.” They do not need to say the word shock; it’s clear that closure of the PDA would cause life-threatening hypoperfusion and thus shock.
Very good point, I agree. This card is directly pulled from a sketchy video (see below), hence my hesitation to change it. But I do think it is asking the wrong question and we should change this card to something like:
What congenital heart disease(s) may cause heart failure and shock after PDA closure?
{{c1::Ductal-dependent congenital heart diseases (e.g. severe coarctation of the aorta, hypoplastic left heart syndrome, transposition of the great arteries)}}
Would somewhat favor having the e.g., be in the extra field just to declutter the cloze.
With a fresh look a couple days later I think maybe:
What congenital heart disease may cause heart failure and shock after PDA closure due to increased afterload?
{{c1::Severe coarctation of the aorta}}
