As you can see under the First Aid Cardio Tags section 23 is missing as well as 29 which according to First Aid is supposed to be “Heart Failure“ different from the “High-output Heart Failure” tag #30…. I reset local settings 2 times, synced and restarted Anki as well and I’m still missing these. Is this intentional or….? There are a ton of other random tags missing like this.

Yes, that is normal. Mostly it has to do with tiny sections of the text that may or may not have anything of note to be tagged (typically an aside, or a short paragraph). Resetting local changes won’t change that, because those tags don’t exist on the AnkiHub side at all and/or have not yet been tagged
